Ps5 broke my disc

I think that my ps4 broke my black ops 3 disc. This morning it worked perfectly and then the servers were down. So I restarted my ps4 and then it said unreconized disc. So I ejected my disc and I look at the back of it, I see in the middle it's like cut in half. I tried putting it back in ps4 and my ps4 just make a weird noise and I hear my ps4 like stop reading the disc, I wanted to see if I put another disc in it it will break so I tried to put my infamous, the disc came back fine. Please help me man, I just bought the new dlc on black ops 3 and all and now I can't even play
