Cannot activate ps4 as primary

I  cannot activate my ps4 as my primary ps4 due to the fact that it says there is already a ps4 activated on my account but I do not own another ps4 nor have I ever let anyone else have my account info. I also cannot deactivate all from the web because it says that I've already used it in the past  6 months even though I know for a fact I haven't.
